What will you need to complete the Mediation Application form:

  1. Full Names, Contact and addresses of the Applicant (yourself) and the Respondent (your spouse)
  2. Statement of Case — a written account of the current situation, including the background to the dispute, any individuals who have been involved or have played a role in the matter, and what you believe a fair and reasonable resolution would look like 
  3. A valid Credit or Debit card to complete the non-refundable registration payment of £20.
